"112th Academic Year Zhang De-Cui Outstanding Academic Scholarship by the Chung-Ta Shu Foundation" - Applications Now Open!

Application Period: From September 1, 2023 (Friday) to September 22, 2023 (Friday) by 5:00 PM.

### Eligibility:
1. Undergraduate and Master's students enrolled in the School of Management with excellent academic and conduct records.
2. Conduct grade of 80 or above with no disciplinary records.
3. Academic grade of 80 or above with all subjects passed.
4. Physical education grade of 70 or above.

### Application Process and Required Documents:
Please prepare and organize the following documents in order and submit them to the School of Management Office (Room 608, Zhi-Xi Building). The school can recommend one candidate for further review by the Student Assistance Division.
1. Application Form printed from the Scholarship Application System. (Log in to the Chung-Ta Portal - Scholarship Application System to print).
2. Autobiography (including a description of family financial status within 500 words).
3. Household Registration Transcript from the past three months or a photocopy of the updated Household Registration Booklet (do not omit the remarks section).
4. Transcript of Academic Records. (For first-year undergraduate or Master's students, a transcript from the previous school may be submitted).
5. Official Proof of Income (submit one of the following):
   - Income statement for 2022 from the National Taxation Bureau for the entire household (father + mother + applicant), and property statement for 2022 from the National Taxation Bureau for the entire household (father + mother + applicant).
   - Low-income household certificate.
   - Middle- to low-income household certificate.
   - Certificate for children from families with special circumstances.

### Number of Awards and Scholarship Amount:
The school can recommend one candidate, with a scholarship amount of NT$10,000 for the selected individual.
